Your Duty BP Check Is Lying: Shift Work Hypertension
You get your blood pressure checked once a year at your department physical, sitting in a chair, probably mid-morning, probably after a decent night of sleep. It comes back “normal.” Meanwhile your actual blood pressure on a real night shift — the one where you ran three calls back to back and ate a gas station burrito at 3 a.m. — never got measured at all. That’s the problem. A single daytime cuff reading tells you almost nothing about what’s happening to your cardiovascular system on the schedule you actually work.
This isn’t a minor technicality. Hypertension is a major driver of the sudden cardiac events that remain the leading cause of duty-related death in the fire service, and it’s just as relevant for law enforcement, corrections, and EMS. If the only number your doctor has is a clean reading from a calm Tuesday morning, you and your provider are both flying blind on the risk that actually matters.
Why One Reading Misses the Real Picture
Blood pressure isn’t supposed to be flat across 24 hours. In healthy people, BP naturally dips 10-20% overnight during sleep — a pattern called “dipping.” Research on night shift workers consistently shows the opposite happening: nighttime systolic and diastolic pressure rise, the normal overnight dip shrinks or disappears, and heart rate stays elevated during hours the body should be recovering. That pattern is called “non-dipping,” and it’s associated with higher cardiovascular risk independent of your daytime numbers.
This creates a specific problem for first responders: you can have a textbook-normal reading in the exam room and still be running elevated, non-dipping pressure on the nights and 24-hour shifts that make up half your working life. That’s masked hypertension — blood pressure that looks fine in the clinic but isn’t fine in the real world. A single annual cuff check is structurally incapable of catching it.
What the Research Actually Shows About Shift Work
Studies on firefighters have found that additional 24-hour shifts in a given month are associated with measurably higher diastolic blood pressure, with the effect growing as shift frequency increases. Police officer studies looking at 12-hour rotating rosters have found similar patterns of disrupted sleep and elevated cardiovascular strain tied to the schedule itself, not just to individual fitness or diet. Across shift-working populations broadly, researchers have found meaningfully elevated cardiovascular disease risk compared with people working standard daytime hours, and shift work has been specifically linked to a higher rate of uncontrolled hypertension even among people already on medication for it.
Put together, this research points at circadian misalignment itself — not just poor sleep or bad food choices — as a driver of blood pressure dysfunction in shift-based occupations. That’s a mechanism a once-a-year physical simply isn’t built to detect.
What to Actually Do About It
You don’t need to wait for your department’s medical program to catch up with the research. A few concrete steps:
- Get a home blood pressure monitor and use it on both shift types. A validated upper-arm cuff (Omron and Withings both make well-tested models) costs under $60. Take readings at the same points relative to a day shift and a night shift — for example, on waking and before your first meal — for two weeks, and log the results.
- Ask your doctor about 24-hour ambulatory blood pressure monitoring (ABPM). This is the gold-standard test for catching non-dipping patterns and masked hypertension, and it’s designed to be worn through a normal working day — including a night shift, if you schedule it on one. If your provider hasn’t heard of ordering it around a 24-hour shift, that’s worth pushing on directly.
- Bring your own log to your physical, not just the day-of reading. A two-week home log showing your numbers on shift days versus off days gives your provider something the annual snapshot can’t.
- Treat non-dipping as a finding, not a footnote. If ABPM shows your nighttime pressure isn’t dropping the way it should, that’s a legitimate reason to discuss medication timing, sleep strategy, or a referral — not something to shrug off because your office number looked fine.
- Don’t wait for symptoms. Hypertension is famously silent. The absence of headaches or dizziness tells you nothing about whether your arteries are under sustained pressure at 4 a.m. on shift four of five.
The Bottom Line
Your job doesn’t run on a 9-to-5 circadian rhythm, and your blood pressure monitoring shouldn’t either. A clean reading at your annual physical is reassuring, but it’s answering the wrong question if it’s only ever measured during your body’s easiest hours. Push for monitoring that reflects the schedule you actually work — because the cardiovascular risk that kills first responders doesn’t show up on a calm Tuesday morning either.
