    “You Don’t Qualify.” A Texas Paramedic Had Low Testosterone and a Climbing TSH for Two Years. His PCP Still Wouldn’t Refer Him.


    The following account was shared with us by an anonymous paramedic from the Dallas-Fort Worth area. We get versions of this story every week — a first responder who knows something is wrong, whose labs back them up, and whose doctor still won’t move. We’re publishing this one because the roadblock he hit is nearly universal, and the fix is more accessible than most people realize.


    I’ve been a paramedic for fourteen years. I know what tired looks like. This was different. I started feeling it about two and a half years ago — brain fog during calls that I couldn’t shake, joint pain in my hips and knees that didn’t match my activity level, a libido that had basically disappeared, and a kind of flat, heavy feeling I didn’t have a word for. Not depression exactly. Just off.

    I got labs done through my PCP. Testosterone came back at 261 ng/dL. TSH was 4.6. She said, “These are within normal range. I wouldn’t be concerned.” I asked her about an endocrinology referral. She told me I didn’t meet the criteria. When I pushed back, she said, “You work a stressful job. A lot of what you’re describing is lifestyle-related.” She suggested I try to sleep more and reduce stress.

    I’m a paramedic who works 24-on/48-off. I run calls for twelve to eighteen hours straight. My TSH was nearly 5. My testosterone was 261. And the recommendation was to sleep more.

    I spent the next six months going in circles. Finally I just went outside the system — found a men’s health telehealth provider, got a full panel done out of pocket, and had a provider who actually looked at the whole picture. Turns out my Free T3 was low, my Free T4 was borderline, and my free testosterone was in the basement even though my total was “normal.” I’ve been on TRT and a low-dose thyroid protocol for seven months. I feel like myself again.

    The wild part is I spent two years being told nothing was wrong. All the information was right there in the labs. Someone just had to be willing to look.

    What Was Actually Happening Clinically

    This paramedic’s story surfaces something that comes up constantly in first responder healthcare: the difference between technically within range and optimal.

    His total testosterone of 261 ng/dL sits at the low end of the standard reference range (typically 264-916 ng/dL). By the letter of most primary care protocols, that’s borderline-normal — not a confirmed diagnosis, not an automatic referral trigger.

    But total testosterone is only part of the picture. Free testosterone — the fraction not bound to sex hormone-binding globulin (SHBG) — is what actually drives the effects of testosterone in the body. A man can have a total testosterone of 400 and a free testosterone so low it produces every classic hypogonadal symptom. Without free testosterone and SHBG, a total testosterone number is incomplete data.

    His TSH of 4.6 is similarly “within range” at most labs, but a significant body of evidence suggests that symptomatic patients with a TSH above 2.5-3.0 deserve a more complete workup: Free T3, Free T4, and in some cases reverse T3 and thyroid antibodies. A TSH alone tells you the pituitary is asking the thyroid to work harder. It doesn’t tell you whether the thyroid is responding, or whether the conversion from T4 to active T3 is happening correctly.

    What his PCP ran was a screening panel. What he actually needed was a diagnostic panel.

    Why This Keeps Happening to First Responders

    Most PCPs are trained to refer when lab values fall outside the reference range — not when a patient’s symptoms suggest suboptimal function within it. That model fails systematically for first responders.

    Shift work suppresses hormone levels. Chronic circadian disruption from 24-hour shifts is associated with lower testosterone, disrupted cortisol rhythms, and altered thyroid function. Reference ranges are built from population samples that skew heavily toward 9-to-5 workers.

    “Normal” ranges are not “optimal” ranges. The standard testosterone reference range includes the bottom 2.5% of tested males. A 35-year-old paramedic at 261 ng/dL is technically in the same category as an 80-year-old with no symptoms.

    PCPs are not trained in optimization medicine. Men’s health and hormonal optimization fall outside the scope of what most PCPs were trained to evaluate — and because referral criteria are defined by diagnosis, not symptoms, a patient without a confirmed diagnosis often gets sent home.

    The system penalizes referrals. Under most managed care structures, PCPs face informal pressure to limit specialist referrals. “You don’t meet criteria” is frequently less about clinical judgment than about how referral behavior is tracked within a health system.

    What a Better Workup Looks Like — and What to Do If You’re Stuck

    A complete testosterone panel should include: total testosterone (before 10 AM), free testosterone, SHBG, LH and FSH, estradiol (sensitive assay), PSA, CBC and metabolic panel.

    A complete thyroid panel should include: TSH, Free T3, Free T4, reverse T3 (if fatigue and brain fog are primary), TPO antibodies and thyroglobulin antibodies.

    If your PCP won’t run this panel or won’t refer:

    4. Ask for labs by name. “I’d like Free T3, Free T4, Free testosterone, and SHBG” is a different conversation than “I feel tired.”

    The Takeaway

    Two years is a long time to feel wrong when the answer was available the whole time. If you’ve been told your labs are “fine” but you still feel like garbage — push harder, or go around. The primary care system wasn’t designed to catch suboptimal function in people whose jobs are actively eroding their baseline.

    You are not otherwise well in the same sense your doctor’s reference ranges assume. That’s physiology, not weakness. The solution is to find a provider who understands that distinction.
