“Your Cortisol Is Fine.” I’d Been Working Nights for Eight Years. My Doctor Ordered One Morning Blood Draw. That’s Not How This Works.

I’ve been a dispatcher for the Sheriff’s Office for eight years. Six of those years have been overnight shifts — 10 PM to 6 AM. I love the work. I hate what it’s done to my body.
For the last two years, I’ve been exhausted in a way that doesn’t make sense. Not “I need more coffee” tired. I mean waking up at 2 in the afternoon after sleeping eight hours and feeling like I hadn’t slept at all. My joints ached. I had brain fog that I kept trying to push through. I gained about twenty pounds despite not eating differently. My mood was flat. My husband noticed before I did.
I finally went to my doctor and described all of this. She ordered a basic metabolic panel, thyroid, CBC. Everything came back normal. She ordered one more test — cortisol — and called me a few days later. “Your cortisol is fine. It’s 18. That’s perfectly normal.” She told me to try to sleep better and exercise more.

It was another eight months before I found a provider who ordered a DUTCH test and a four-point salivary cortisol panel. What came back wasn’t a single number. It was a picture of a rhythm that was completely inverted. My cortisol was high at night — spiking when I should have been winding down — and barely registering in my morning hours. What Actually Happened: Why a Single Morning Cortisol Draw Is Nearly Useless for Shift Workers
Cortisol is a hormone produced by the adrenal glands that follows a predictable diurnal rhythm — for people who sleep at night. In a normal sleeper, cortisol is highest in the first 30 to 60 minutes after waking (the “cortisol awakening response”), gradually drops through the day, and bottoms out at night. A blood draw at 7 to 8 AM will catch that morning peak and give a result that looks robust.
For someone who has been working nights for years, that rhythm inverts. Research on shift workers consistently shows that chronic night shift schedules disrupt the HPA (hypothalamic-pituitary-adrenal) axis, alter the cortisol awakening response, and in many cases produce a flattened or phase-shifted cortisol curve. Multiple occupational health studies have documented elevated nighttime cortisol and blunted daytime cortisol in long-term shift workers — patterns directly associated with metabolic dysfunction, weight gain, fatigue, immune suppression, and mood dysregulation.
The dispatcher’s doctor was technically correct that a serum cortisol of 18 mcg/dL is within the standard reference range for an 8 AM morning draw in a day-shift person. What she missed is that this reference range assumes a normal sleep-wake cycle. Drawing a morning cortisol from a night-shift worker who has been awake all night is like testing blood sugar two hours after a meal and comparing it to a fasting reference range. The number exists. The comparison is meaningless.
Why This Keeps Happening
There are a few structural reasons why primary care medicine keeps failing shift workers on cortisol and adrenal testing.
Reference ranges were built on day-shift populations. The lab’s reference range for morning cortisol (typically 6–23 mcg/dL) was established from studies on people with normal sleep schedules. There is no widely-used adjusted reference range for rotating or permanent night-shift workers, even though this population numbers in the tens of millions in the United States.
Single-draw cortisol testing is inadequate for anyone concerned about adrenal or HPA function. A single number tells you almost nothing about rhythm. Cortisol is dynamic — it should be high in the morning, drop through the day, and hit a nadir at night. A single morning result can be “normal” even in someone with a completely dysregulated diurnal pattern because it captures one data point.
Primary care physicians are not trained in functional endocrinology or chronobiology. Most PCPs receive minimal training in how to assess HPA axis function in shift workers. The DUTCH test, four-point salivary cortisol, and other rhythm-based assessments are standard tools in integrative and functional medicine but are rarely ordered in conventional primary care settings.
There’s a built-in scheduling problem. Even a doctor who wanted to order a proper four-point salivary cortisol panel would have a hard time telling a night-shift worker to “collect your sample at 8 AM, noon, 4 PM, and bedtime.” Those instructions assume a daytime life. Getting this right requires a provider who will actually ask about your schedule and calibrate the testing protocol to your reality.
What a Better Workup Actually Looks Like
If you’re a night-shift first responder experiencing chronic fatigue, weight gain, brain fog, poor sleep quality, or mood changes that standard labs haven’t explained, here’s what a more appropriate workup looks like:

- Ask your provider to account for your circadian reality. Any provider ordering cortisol testing on a shift worker should document your schedule and adjust interpretation accordingly. If they don’t ask what shift you work, that’s a red flag.
- Functional and integrative medicine providers. These are the practitioners most likely to be trained in DUTCH testing, HPA axis evaluation, and chronobiology. Many work through telehealth, which matters for first responders who can’t take a Tuesday morning off work.
- Don’t accept “your cortisol is normal” from a single morning blood draw alone. Especially if you’ve been working nights for years. Ask what test was run, when it was drawn relative to your wake time, and what the reference range assumes about your schedule.
The question isn’t just whether your cortisol number falls in a range. The question is whether your cortisol rhythm is functioning the way it’s supposed to — and after six or eight years of night shifts, the honest answer is: probably not entirely.
That rhythm can be assessed, supported, and in many cases improved with targeted interventions: light therapy, sleep hygiene protocols calibrated to shift work, adaptogenic supplements like ashwagandha and phosphatidylserine, and in some cases low-dose hormonal support. But none of that starts until someone actually runs the right test.
