    “Your Levels Are Fine.” A Firefighter’s Testosterone Was 315. His Doctor Wasn’t Worried. He Should’ve Been.


    The following account was shared with us by an anonymous firefighter in California. We hear versions of this story constantly. We’re publishing it because it deserves to be said out loud.

    “I asked my doctor for a testosterone test and it got weird. He looked at me like I was drug-seeking. ‘Why would you be worried about that?’ he said. Then, ‘You’re pretty muscular.’

    Like being muscular means your hormones can’t be tanked. Like the only reason a man would want his testosterone checked is because he’s trying to score something.

    My level came back at 315. Eight in the morning — which is when testosterone peaks. The doctor told me I was perfect. ‘Normal range is 300 to 750,’ he said.

    I asked him: if 315 is what I’m running at 8am, what am I bottoming out at when it’s bedtime?

    He didn’t have an answer for that. Just told me to get more sleep.

    This is one of the most common stories we hear from first responders. And there are a few things in it that deserve to be unpacked — because they point to a gap in how mainstream medicine handles this population.

    The “Normal Range” Problem

    315 ng/dL is technically within the normal reference range for total testosterone. That range — typically cited as 300–1,000 ng/dL depending on the lab — is a population average. It represents the spread across all adult men, from 18-year-olds to 80-year-olds, sedentary and active, healthy and not.

    Being “within range” tells you almost nothing about whether your testosterone is appropriate for you, your age, your activity level, and your symptoms.

    What it also doesn’t tell you: where you are in the day. Testosterone follows a diurnal rhythm. It peaks in the early morning hours — typically between 6 and 10 AM. By evening, it’s dropped 20–35% from that morning peak. In some men, especially those with disrupted sleep or high cortisol, the drop is steeper.

    So a morning reading of 315 — already near the floor of the “normal” range — is not the full picture. That number at 8 AM suggests evening levels that may be closer to 200. Maybe lower.

    A physician who understands hormones looks at a morning 315 in a symptomatic first responder and treats it as a flag, not a reassurance. A physician who doesn’t understand the context of shift work looks at it and says you’re fine.

    Why the Suspicion?

    This part of the story is worth addressing directly, because it’s not unique to this firefighter.

    There is a persistent clinical assumption that men who ask about testosterone — especially men who appear physically fit — are either seeking anabolic steroids or engaging in some kind of self-improvement vanity project. The concern isn’t totally without basis in the history of hormone misuse. But it’s resulted in a reflexive gatekeeping behavior that works against the men who actually need care.

    A firefighter asking about testosterone is asking because he’s exhausted in ways that don’t respond to sleep. Because his recovery from training has collapsed. Because he’s short-tempered in ways that are affecting his family. Because his drive for anything — the job, his marriage, his hobbies — has gone flat and he can’t explain why.

    None of that is drug-seeking. It’s a person describing clinical symptoms and asking for an appropriate diagnostic workup.

    “You’re pretty muscular” is not a clinical assessment. Muscle mass does not protect testosterone. In fact, the physical demands of the job — combined with sleep deprivation, chronic cortisol elevation, and circadian disruption — are part of what drives testosterone down in this population.

    What Shift Work Does That Your Doctor Probably Didn’t Ask About

    Here’s the physiological context that a 15-minute primary care appointment almost never includes.

    Testosterone production depends on sleep. The largest pulse of luteinizing hormone — the signal that tells your testes to produce testosterone — happens during deep sleep. Specifically slow-wave sleep, in the early morning hours. When you’re working 24-hour tours, rotating from days to nights, and sleeping at irregular hours in a fire station with a tone that can drop at any moment, you are not getting consistent deep sleep. The LH pulse doesn’t fire the way it should. Testosterone production suffers.

    Shift work suppresses LH directly. Research on shift workers across multiple industries has found that circadian disruption doesn’t just affect sleep — it affects the hormonal cascade upstream of testosterone production. The hypothalamic-pituitary-gonadal axis is sensitive to circadian cues. Disrupt the clock, disrupt the output.

    Cortisol and testosterone are in direct competition. Both are downstream of pregnenolone — a precursor hormone. When your stress load is high and sustained (which operational first responder work produces reliably), your body preferentially synthesizes cortisol at the expense of testosterone. This is sometimes called pregnenolone steal. It’s not a theory — it’s a documented biochemical pathway that explains why chronically stressed people present with low testosterone even without other obvious causes.

    A doctor who doesn’t ask about your shift schedule, your sleep quality, and your stress load before reviewing your testosterone labs is missing the clinical context that makes those numbers interpretable.

    What a Better Workup Looks Like

    If you’re a first responder with symptoms of low testosterone — fatigue, slow recovery, mood changes, loss of drive, body composition shifts toward fat — here is what a thorough diagnostic workup includes. Not a total testosterone blood draw at 8 AM and a “you’re fine.”

    Labs worth requesting:

    • Total testosterone (morning draw, before 10 AM)
    • Free testosterone (either calculated or direct)
    • SHBG (sex hormone-binding globulin — high SHBG can make a borderline total T clinically low)
    • LH and FSH (tells you whether the problem is the testes or the signaling from above)
    • Estradiol (E2)
    • Complete metabolic panel
    • CBC
    • Thyroid panel (TSH, free T3, free T4)
    • PSA if over 40

    Context the provider should be collecting:

    • Shift schedule and how long you’ve been working it
    • Sleep quality and quantity (not just hours in bed — quality matters enormously)
    • Stress history and current operational tempo
    • Symptoms timeline — when did things change?

    A provider who doesn’t ask these questions before interpreting your labs is not giving you a complete picture.

    What Happens Next

    For the firefighter in California — and the thousands of first responders in the same position — there are two realistic paths.

    Path one: Go back to a primary care physician who views a 315 as normal, get told to sleep more and exercise, and spend another few years feeling like something is wrong with you that you can’t explain.

    Path two: Find a provider who understands occupational context, does a complete workup, and has a genuine conversation about whether your symptoms and labs together justify intervention.

    That could mean testosterone replacement therapy — weekly injections, gels, or pellets — under proper medical supervision. It could mean addressing sleep and cortisol first and retesting. It could mean a combination. What it doesn’t mean is dismissal.

    The telehealth men’s health space has expanded significantly in the last five years, and a growing number of platforms understand the first responder picture specifically. You don’t need to convince them that your job might be affecting your hormones. They already know.

    The Actual Answer to His Question

    He asked: if 315 is where I’m peaking at 8 AM, what am I bottoming out at when it’s bedtime?

    The answer is: lower. Possibly significantly lower. And in a man who works the schedule he works, carries the stress load he carries, and sleeps the way he sleeps — that’s not a curiosity. That’s a clinical question worth taking seriously.

    The fact that it wasn’t treated as one isn’t a reason to stop asking. It’s a reason to find a different provider.
