Blood Donation, PFAS, and Ferritin: A Firefighter Guide
Short answer: Yes — regular donation measurably lowers PFAS in firefighters. In a randomized trial of 285 Australian firefighters, plasma donation every six weeks for a year dropped mean PFOS levels roughly 30%, and whole blood donation every 12 weeks dropped them roughly 10%, while the non-donating control group didn’t budge. Why This Matters for First Responders
Firefighters as an occupational group carry higher blood PFAS levels than the general public — a legacy of AFFF foam, combustion byproducts, and PFAS-treated turnout gear. Everything else in the exposure conversation is defensive: store gear outside living quarters, launder on schedule, decon early. Donation is the only lever anyone has demonstrated that actually lowers the number already in your blood.
The iron side matters for a different population. Male first responders in their 40s and 50s who eat a heavy red-meat diet, drink more than they’d admit on a physical, and carry an HFE variant they’ve never been tested for are exactly the people who show up with a ferritin of 600 and get told “it’s probably inflammation.” Sometimes it is. Sometimes it’s the start of liver, joint, and cardiac damage that a couple of donations a year would have prevented.
Practical scheduling note: a whole blood donation temporarily drops your hemoglobin and can blunt hard efforts for a few days. Second, the two goals pull in opposite directions. The donation type best at clearing PFAS is the one that does nothing for iron, and the one that clears iron fastest wasn’t tested for PFAS at all. If your ferritin and transferrin saturation are normal and your only concern is occupational PFAS, plasma is the more efficient choice and you can do it more often. If your ferritin is genuinely elevated with high transferrin saturation, whole blood or Power Red is the right call and the PFAS benefit is a bonus. - Get a full iron panel, not just ferritin: ferritin, serum iron, TIBC, and transferrin saturation, plus a CBC.
- If ferritin is high and saturation is normal, look for the real driver — liver enzymes, A1c, alcohol intake, inflammatory markers.
- If ferritin is high and saturation runs above ~45% on repeat testing, ask for HFE genotyping before you start donating on your own.
- If your labs are clean and PFAS is your concern, book plasma donation on a recurring 28-day cadence and keep it up — the trial effect came from a year of consistency, not a one-time gesture.
- Recheck ferritin and hemoglobin at 6 and 12 months. Donation is a dose, and doses need monitoring.
