The Cortisol Test Your Doctor Isn't Running (And Why It Matters for First Responders)
Why a single morning cortisol draw misses what shift work is actually doing to your HPA axis.
If you've ever mentioned fatigue, weight gain, mood problems, or low libido to a primary care doctor and they actually checked your cortisol, they probably ordered one thing: a serum cortisol blood draw. Single time point. Usually morning.
The number came back. They said it was normal. End of conversation.
Here's the problem with that: a single cortisol blood draw is one of the least useful ways to evaluate cortisol function in a shift worker. It's the right test for diagnosing Cushing's disease or Addison's disease — serious conditions where cortisol is dramatically, constantly elevated or suppressed. It is not built to catch the kind of cortisol dysregulation that accumulates over a career in fire, law enforcement, or EMS.
The test that actually captures what's happening to first responders is called the DUTCH test. Most primary care physicians have never ordered one. Many haven't heard of it. That gap in awareness is leaving a lot of people without answers they could have.
Why Single-Point Cortisol Testing Misses the Picture
Cortisol isn't a static number. It follows a steep daily curve — called the diurnal rhythm — that is supposed to look something like this: a sharp spike at waking (the cortisol awakening response, or CAR), a gradual decline through the morning and afternoon, and a near-floor level by bedtime that allows melatonin to rise and sleep to begin.
This rhythm is not just incidental. It's load-bearing for your energy, your immune function, your mood, your sleep, and your hormone balance. When the rhythm breaks — when the curve flattens, inverts, or peaks at the wrong times — you feel it. But a single morning blood draw can't show you a curve. It shows you one point on a graph and tells you nothing about the shape of the line.
For a firefighter or cop working rotating shifts, the curve is almost certainly abnormal. The question is how and where — and that's what proper testing can tell you.
What the DUTCH Test Actually Is
DUTCH stands for Dried Urine Test for Comprehensive Hormones. It was developed by Precision Analytical, a lab in Oregon, and has become the gold standard in functional and integrative medicine for evaluating hormonal health.
You collect urine samples at four to five points throughout the day and into the night — on a filter paper strip that dries and gets mailed to the lab. The timing captures your hormonal rhythm across the full day rather than a snapshot.
What it measures that a serum cortisol draw doesn't:
Cortisol pattern across the day. Not just the level — the rhythm. The DUTCH shows your cortisol awakening response (which should be a robust spike within 30–45 minutes of waking), your midday level, your afternoon level, and your evening floor. First responders commonly show a blunted CAR, which correlates with burnout and HPA axis suppression — or an elevated evening cortisol, which destroys sleep and tanks testosterone.
Cortisol metabolites. The liver processes cortisol into metabolites (tetrahydrocortisol, tetrahydrocortisone, and others). The ratio and quantity of these metabolites tells you whether the problem is production, clearance, or conversion — distinctions that matter for treatment. High cortisol production with fast clearance looks different from high cortisol production with slow clearance, even if the active cortisol level appears similar.
DHEA and DHEA-S. DHEA is the counter-regulatory hormone to cortisol — they're supposed to balance each other. In chronically stressed individuals, DHEA typically declines while cortisol stays elevated. Low DHEA is associated with faster aging, immune dysfunction, low libido, and depression. Most doctors never check it.
Sex hormones. The DUTCH includes estrogen, progesterone, and testosterone metabolites, giving a complete picture of how cortisol dysregulation is affecting the broader hormonal environment. Because these pathways are interconnected — cortisol, testosterone, and estrogen all share upstream precursors — seeing them together is more useful than seeing any one in isolation.
Melatonin (in the extended version). The DUTCH Complete includes a melatonin marker, which can show whether your sleep issues are driven by melatonin suppression — directly relevant to shift workers under artificial light.
Organic acids. Markers for B vitamin status, neurotransmitter metabolism, and oxidative stress. Useful context for the full clinical picture.
What First Responder Cortisol Patterns Actually Look Like
Based on what's consistently seen in this population — and what the functional medicine providers who work with first responders report — there are a few common DUTCH patterns:
Blunted cortisol awakening response with normal or elevated daytime cortisol. This is classic HPA axis fatigue — sometimes called adrenal fatigue, though that term is contested. The awakening spike that's supposed to prepare you for the day is flat or absent, so you wake up already dragging. But because daytime cortisol is maintained (your body is compensating), a standard blood draw might look normal.
Elevated evening cortisol. This is the pattern that kills sleep. Your evening cortisol should be near zero — instead it's high enough to block melatonin, keep your brain alert when it should be winding down, and suppress the deep sleep cycles where testosterone and growth hormone are produced. Often correlates with the experience of being exhausted but wired at night.
Low DHEA with variable cortisol. The ratio matters more than either number alone. Low DHEA relative to cortisol suggests the adrenal glands are prioritizing stress hormone production over everything else — a pattern linked to accelerated biological aging and immune dysfunction.
Elevated cortisol metabolites with normal active cortisol. The liver is processing cortisol rapidly, which means total cortisol production is higher than the active level suggests. This can be missed entirely on a blood draw.
How to Actually Get a DUTCH Test
Here's where it gets practical, because this is the part that trips people up.
Primary care almost never orders it. The DUTCH is a specialty test run by a specific lab. Most primary care physicians aren't familiar with it, don't know how to interpret it, and work within insurance billing systems that don't accommodate it easily. Pushing for it through a GP will usually be a dead end.
Functional medicine and integrative medicine providers order it routinely. If you can find an integrative MD, DO, or functional medicine-trained NP in your area, DUTCH testing is often part of their standard intake. These providers also know how to interpret the results and build a protocol around them.
A growing number of telehealth platforms include it. This is where the shift is happening fastest. What Happens After the Test
A DUTCH result without a competent provider to act on it is just data. Here's what an appropriate clinical response to common first responder patterns looks like:
Blunted CAR: Phosphatidylserine, adaptogens (ashwagandha, rhodiola), aggressive sleep hygiene, sometimes low-dose hydrocortisone in supervised protocols. Light therapy immediately on waking. Evaluating for and treating underlying sleep disorders.
Elevated evening cortisol: Ashwagandha (KSM-66), magnolia bark, phosphatidylserine taken in the afternoon, blue-light avoidance after sunset, blackout sleep environment, sometimes low-dose melatonin for circadian signaling. Eliminating caffeine after noon.

Why This Matters More Than It Sounds
First responders normalize feeling bad. The job is hard, the schedule is brutal, the stress is real — and most people in this profession are highly resistant to admitting that their body is struggling. "Tired" becomes a baseline. "Off" becomes acceptable.
The DUTCH test doesn't change the job. It doesn't fix the schedule. What it does is give you — and a competent provider — a precise picture of what your career is doing to your hormones, so you can intervene with something more targeted than "get more sleep and try to stress less."
A single morning cortisol draw and a shrug is not a workup. It's a guess. You deserve better than a guess.
